I have been sent an appointment for an adult autism assessment on May 3rd, think it is the interim assessment, and not a final one. They have sent me the AQ-10, and a behaviour questionnaire for me to fill in, and ingeniously have sent me a relative's questionnaire to be filled in before the day-my parents live 200 miles away and unless you pay for special delivery there is a big risk of it not getting to them and back in time. I tried to find an online pdf or doc of the questionnaire but couldn't find one. Any suggestions? (I also worry that my parents won't remember me between the ages of 4-10 accurately but that is a whole other issue).
 
My dad suggested I scan the questionnaire, him and mum print it and fill it in and send it special delivery. So that is that taken care of at least. I realise the behavioral questionnaire is either the EQ on it's own or the EQ/SQ combined test under a different name.
 
I was given the SCQ for my parents to fill in less than a month ago. I suggest you print the DSM-V symptomatology if you are more comfortable, if you want to tell the person you're meeting things and so you don't forget things that you want to tell him :)

Thank you :) it is a female 'practitioner' (that is what the letter says), definitely more comfortable with that than a male. Some areas in the uk use the DSM-5, some use the WHO version still, haven't had a straight answer on what they use here.
 
Update, I got the completed relatives' questionnaire back from my parents, while they seem to have answered a couple of questions no when that is not correct, the majority of how they have answered seems correct and they annotated some of their answers to clarify-which is helpful.
