There's probably a language where Metal Head, is an idiom for something (bad) .

"Brick head" is an insult here, although not a common one, and kind of old-fashioned.
I've never heard "metalhead" used to mean anything other than someone who listens to heavy metal music. But I suppose it could translate to something in another language.
